It's a:
GE12PB Steel / Phosphor Bronze Spherical Plain Bearing 12x26x16x12mm.
I got mine from Simply Bearings (UK), cost is £22 delivered.
Unlike the standard bearing it's got a central grease channel, so I've fitted a grease nipple to the crescent. Fitting the bearing was simple enough with a suitable sized drift.
Don't be tempted to fit an IKO GE12G. They're cheaper, but don't have a wide enough central support.
